Urgent Insurance Reporting

This form must be used to notify BSTF of any incidents which might lead to insurance claims (within 5 days of incident) at BSTF events, or for club training if your club holds an activated a BSTF club insurance policy. For club training incidents contact both your SU/ sports staff to report it as well as BSTF via this form (if all of your club members hold the same and only a 3rd party insurance policy e.g., UKGT membership, contact both your SU/ sports staff and your 3rd party organisation’s office).
